D&D 4th Edition - Death

In the new D&D 4th Edition Death has nothing to do!

Healin Surges are the bane of the GM who want life to be exciting and to do that the PC's need to be in fear of at any moment being killed by whatever is around the next corner.

They enter a fight with ....lets say a bugbear, it does its part and fights to the death and a couple of heros are injured, the fight is over so the PC's take a healing surge and they are at full health now for the next fight and so and so on.

The only way I as a GM can stop this is make more and mor encouters for them to get into a fight with until they have used all their Surges. This will never make for a fantastic game if after every fight they are helthy and ready to go instead playing in a more ... how can i put this.... well this is a Role playing game so to play your injury make the game itself exciting, having to rely on your friends in the party to take care of you as you continue into the dungeon or they have to help you get out of the dungeon to het proper help.

So what in the end is the piont of a HEAL skill if everyone just has to use a Surge. If you can suggest how to get the fun into death then please tell me because as a GM this is my downfall.
